Appointments
To schedule an appointment, please call our office at (214) 496-9700 during regular business hours.
Main Office Hours: Monday-Friday 8:00am to 5:00pm (except holidays)
Satellite Office Hours: Monday 1:30pm - 5:00pm.
If you experience a life-threatening emergency, PLEASE DIAL 9-1-1 IMMEDIATELY.
Cancellations
Please call 24 hours in advance if you must cancel an appointment so we may open the schedule for other patients who wish to be seen.
New Patients - Please arrive 15 minutes prior to your scheduled appointment time to complete the necessary registration forms.
After Hours Emergencies
If you are experiencing a medical emergency, our physicians recommend that you Dial 911. However, if you need after hours advice for a medical concern, please call (214) 496-9700, and our answering service will assist you.
Prescription Refills
We only renew prescriptions that we initiated. For other medications, please contact the health professional who prescribed them.
We can fill many renewal requests by phone. Please contact our office at least 48 hours (2 days) before you expect to run out of medication. Sometimes, for medical reasons, we will need to see you before issuing the prescription.
Medical Records
A medical release waiver must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider. If you are requesting that your records be sent to another provider we ask that you supply us with their name, address, phone number, and fax number.
